Terms and Conditions of the Neuter Now Program

TERMS AND CONDITIONS WITH EBT CARD

  • See the Neuter Now brochure for a list of participating veterinarians or touch the link (above, right). Participation by veterinarians is voluntary and they are providing their services at greatly reduced rates.
     
  • Call a participating veterinarian and
    1. verify their participation in the Neuter Now program.
    2. make an appointment for the pre-surgical examination and surgery.
    3. present this certificate and show your EBT card and a government-issued picture I.D. at the veterinarian's office as payment for the Neuter Now surgery.
    4. comply with all instructions given by the veterinarian before and after surgery.
    5. Medication or any services required for the treatment of any post-surgical complications arising later than 24 hours after the surgery are not included and are the responsibility of the owner. Other than these, no other charges may be added.
       
  • Certificates are valid for three months from date issued. If your animal is medically unable to be sterilized within the three-month period, please return the certificate to the Hawaiian Humane Society for a retund within the three-month period.
     
  • Lost certificates will not be replaced, refunded, nor are they transferable.
     
  • Call the Hawaiian Humane Society at 946-2187, ext. 227 if you have any questions.
TERMS AND CONDITIONS without EBT Card

  • Additional services may be required by some veterinarians, and the fees for the services vary. It is recommended that you inquire about services and fees before making an appointment for your animal's surgery.
     
  • Call a participating veterinarian and
    1. verify their participation in the Neuter Now program.
    2. make an appointment for the pre-surgical examination and surgery.
    3. comply with all instructions given by the veterinarian before and after surgery.
